Governors State University is partnering with Med+Proctor to submit your immunization record.
- Visit www.medproctor.com
- Click "register" and create an account in MedProctor using your GSU email address
- Complete your MedProctor Profile
- Download and Print the Immunization Certificate
- Take the Immunization Certificate Form to your medical provider for completion
- Upload an image of the the completed Immunization Certificate form to MedProctor. You may also upload additional documents to verify immunization compliance.
Med Proctor is available 9 a.m. through 5 p.m. central time for assistance, and also offers a live chat feature at support.medproctor.com. You may also email help@medproctor.com.

Governors State University is required by law to comply with the guidelines set by the College Immunization Code (77 III.Admin.Code 694), which requires all students born on or after January 1, 1957 who enroll for 6 credit hours or more to provide the University with proof of immunity against certain communicable diseases.
Effective July 21, 2016 updates to the Illinois General Assembly Administrative Code have increased the immunization requirements for incoming Fall 2016 and forward, college students attending all Illinois State schools. These changes have increased the requirements for Tdap, Measles, Mumps, Rubella, and Meningitis.
All GovState students should log in to myGSU Self Service to check their current Immunization Status. For students who do not have a complete immunization record on file with the Office of the Registrar, a registration hold will be placed on your account and you will not be eligible to register for the following semester. Students should stop by the Registrar's Office before their immunization appointment, if utilizing GSU's Health Center. |
Where can I locate my immunization documentation?
- Previous high school or four-year college/university
- Military records or World Health Organization (WHO)
- Family physician or childhood records
- IDPH - Illinois Comprehensive Automated Immunization Registry Exchange (I-CARE): new online portal to request immunization records is now available for residents aged 18 and older receiving vaccinations in Illinois. Please use this link to retrieve your immunization record: https://idphportal.illinois.gov/s/?language=en_US.
If none of the above options have your records, request an immunity profile (Titers blood test) that confirms your immunity to Measles, Mumps and Rubella. The lab report(s) must be attached. If you are unable to obtain proof of immunization, you will need to be re-vaccinated.
International students, born outside of the United States, must have had a Tuberculosis (TB) skin test in the U.S. within the last 12 months. If records are not in English, they must be accompanied by a certified translation.
- Students admitted Fall: September 1
- Students admitted Spring: February 1
- Students admitted Summer: July 1
Immunization documents must be submitted by the deadlines listed above. Students not in compliance with their immunization records by the above respective deadline will have an Immunization (IMM) Hold placed on their account for future class registration and cannot add, drop or make any schedule changes until compliance is met. Students who are not compliant by the grade period - 1st week in October (for Fall admits), 1st week in March (for Spring admits), or 1st week in August (for Summer admits) - will also have a non-refundable $25.00 late fee assessed.
*Please note that resident students will not be permitted to move into University Housing until immunization compliance is met.

Notice about the Meningococcal Conjugate Meningitis Vaccination
Governors State University, consistent with the American College Health Association (ACHA) and Centers for Disease Control and Prevention – Advisory Committee for Immunization Practices, recommends that students who live in the residence hall and those older than 21 years of age consider the meningococcal conjugate vaccination to reduce their risk for potentially fatal meningococcal disease. For additional information, please refer to the index of the ACHA and the Centers for Disease Control and Prevention. This statement is intended to promote public health awareness and practices, and is in compliance with the Illinois Meningitis Information Law (110 ILCS 690/35-120).
